Guardini crashed early on in the 182km stage, but placed himself well to make a late charge in a chaotic final sprint to edge out Yohann Gène (Europcar) and Davide Cimolai (Lampre-Merida).
Wind and rain played havoc on the stage, with Guardini crashing on the penultimate lap.
Tom Dumoulin had broken free of the peloton and looked like he might hang on with 300m to go, but he was overhauled and ended up 7th.
Guardini takes the overall lead by four seconds thanks to the 10 bonus seconds he gained as stage winner.
